Client agreement & liability waiver

1. Services. 600 Pounds Down and Justin Willoughby ("Coach") provide health, fitness, mindset, and accountability coaching. Coach is not a physician, dietitian, therapist, or licensed medical provider. Nothing provided is medical, psychological, or nutritional treatment or advice.

2. Medical clearance. You confirm you are medically cleared to begin a nutrition and exercise program, and you agree to consult your physician before starting and if any condition changes.

3. Assumption of risk & release. Physical activity and dietary change carry inherent risk, including injury or, in rare cases, death. You voluntarily assume all such risk and release and hold harmless Coach, 600 Pounds Down, and its representatives from any claim, liability, or damages arising from your participation, to the fullest extent permitted by law.

4. All sales are final. All payments are non-refundable and non-transferable. No refunds are issued for missed sessions, changes of mind, unused portions of a program, or discontinued participation.

5. Payment obligation. This program is billed month to month at $199.99 per month. Your card is charged each month until you cancel before your renewal date. Payments already made are final and non-refundable. Failure to pay an incurred charge is a breach of this agreement, and unpaid balances may be sent to collections.

6. Discounts. No paid-in-full discount is offered for this program.

7. Your commitment. Your payment is an investment in your health. You agree to participate honestly, communicate openly, and follow the plan you are given.

8. Privacy. Your intake information is kept confidential and used only to coach you.

9. Electronic signature. Typing your name below constitutes your legally binding electronic signature and acceptance of this agreement.
